Parent/Guardian Consent Policy

Consent Requirements

The Beyond Breakfast Foundation requires written consent from a parent or legal guardian for all participants under the age of 18. Consent must be provided before a minor can participate in any program activities, events, or services.

What Consent Covers

Parent/guardian consent is required for: program enrollment and participation, emergency medical treatment (if needed), transportation to/from program locations (when applicable), communication between mentors and the minor mentee, collection and use of personal information, and participation in group discussions and journaling activities.

Parent/Guardian Rights

Parents and guardians have the right to: review their child’s program records and progress reports, meet with their child’s assigned mentor, attend monthly check-in sessions, withdraw consent and remove their child from the program at any time, request modifications to their child’s participation, and access and correct any personal information we hold about their child.

Communication

Parents/guardians will receive: monthly progress notes from their child’s mentor, notification of any safety concerns or incidents, advance notice of events, field trips, and schedule changes, and access to our parent resource library and support materials.

Revoking Consent

Consent may be revoked at any time by submitting a written request to [email protected] or by contacting the Program Director directly. Upon revocation, the minor’s participation will be concluded, and their personal data will be handled in accordance with our Privacy Policy.
